Pregnant, schizophrenic and distressed Becky is brought into the ED. Her waters have broken and she's in the early stages of labour. Jack's priority is to make sure her baby is delivered safely. When husband Joe arrives she becomes even more distressed - "don't let him kill my baby!" Joe describes his wife's history of mental illness and how they tragically lost their first baby at full term. Becky has been fine until recently when she got rid of her medication. If anything happens to this pregnancy then their lives will be ruined. Jack tries to balance care for Becky without compromising the healthy delivery of her baby, no easy trick.

Finn is a heroin addict who's been bashed and brought into the ED. He is uncooperative and abusive. Zoe and Steve treat him. There is no way they'll give him opiates to manage his pain. X-rays reveal fractured ribs and pneumonia. Steve is happy to ship him out of the ED so he's someone else's problem. However when tests come back confirming PCP (Pneumocystis carinii Pneumonia) it is clear that Finn is very unwell. When Steve confronts him about his HIV status he realises that Finn has no idea how sick he is and Steve gets to see the scared young man behind the emotional scar tissue.

Zoe and Steve clash over the treatment of Finn after Steve writes up morphine to relieve his pain. Frank sides with Steve leaving Zoe's nose out of joint. Frank and Steve tell Finn his only option is to be sedated and ventilated but even then his prognosis is poor. Finn decides to refuse treatment. Steve, initially reluctant to accept this decision has a turnaround and stays with him until the end. Steve tries to grant Finn a final wish which brings him face to face with his own addiction.
Jack tells Gabrielle that he'd like to accept her offer of accommodation. Steve is less than thrilled.

Bart and Charlotte spend the day doing an Emergency Simulation Course. Frank helps set them up and they humourously crash and burn. Afterwards Frank tells Charlotte she's received her Fellowship.
